IRA Database Project
India lacks a database for common rheumatic diseases, without which we depend on the western description for our patients. The clinical presentation, complications, treatment response, and outcome of rheumatic diseases in India are therefore unknown. Although COPCORD studies have generated a few community-based symptom-related databases, the diagnoses are largely unknown or underestimated. With this background, we intend to generate a database to understand the clinical presentation and outcome of ten select common rheumatic diseases in referral centers setups across the nation, henceforth termed as ‘IRA database project.’ The two specific objectives of the project would be:

1. To characterize the clinical presentation and demographics of common rheumatic diseases

2. To assess the treatment outcomes of common rheumatic diseases using existing protocols in referral centers setups in India

Data for the project will be captured in a computerized record form (CRF) drafted individually for each of the 10 common rheumatic diseases: RA, spondyloarthritis, gout, Sjogren’s syndrome, SLE, scleroderma, myositis, MCTD, ANCA vasculitis, and Takayasu arteritis. Individual center data can only be accessed by that center, and it will not be possible to access data of other centers. Central database generation and data management will be done by the Department of Biostatics, Christian Medical College and Hospital, Vellore. For the same purpose, we invite interested centers with electronic record systems and their proposed principal investigators (PIs) to participate in the project. Interested centers and PIs can contact us at the following address for further details.

Overview of Biosimilar Conclave

Location: New Delhi
Date: 20 January 2019

Biologics are considered the next generation of treatment for many life-threatening and rare illnesses. They are produced by cells in culture or whole organisms; this is inherently more variable than chemical synthesis methods. Therefore, unlike generic pharmaceuticals, it is impossible to generate the same or identical copy of an innovator product. In this way, biosimilars are identical but not the same to the original biologic innovator product. Therefore, the field of biosimilars presents several important challenges, including verification of the similarity, the interchangeability of biosimilars and innovator products, the possible need for unique naming to differentiate various biopharmaceutical products, regulatory framework, commercial opportunities, as well as guidelines to assist manufacturers in product development and public safety.

To address these issues, the Arthritis and Rheumatic Disease Foundation (ARDF) in association with Zydus Biovation (Cadila Healthcare Ltd.), conducted an academic event entitled ‘Biosimilars Conclave’ on 20 January 2019. The speakers and chairpersons were experienced rheumatologists from the length and breadth of India. They discussed the various biosimilars in rheumatology available in India; the DCGI was represented by an Assistant Drugs Controller, who provided an overview of the regulatory framework for biosimilars in India, which reassured about the rigorous process for approval of biosimilars in India. He also answered queries regarding the conduct of investigator-initiated studies in India. The research and development of biosimilars were described by the head of the biologicals research division of Zydus. The session on the future road map was laid out effectively with an emphasis on quality and cost.

The event concluded with a consensus statement from Dr. Amita Agarwal, that biosimilars would be a cost-effective treatment option for various rheumatological diseases, despite the stringent regulations. However, robust post-marketing surveillance in the form of registries is required.
